Prior to your first appointment you should obtain records from previous dental provider if possible, particularly xrays; release form provided. Please also be ready to provide a list of current medications and medical conditions; form provided to fill out prior to appointment if you choose.

What to expect:
At your new patient visit we will obtain current medical information including medication list, current xrays, document current conditions including any needed treatment, capture new patient photos, and if time complete a cleaning if indicated.
Payment is required on the day of service. We do not participate or accept any insurance. We will however, bill the insurance company for the patient, and the patient gets reimbursed from the insurance company.
Any questions regarding billing and / or insurance coverage should be directed to the front desk.
