Prior to your first appointment you should obtain records from previous dental provider if possible, particularly xrays; release form provided. Please also be ready to provide a list of current medications and medical conditions; form provided to fill out prior to appointment if you choose.

What to expect:
At your new patient visit we will obtain current medical information including medication list, current xrays, document current conditions including any needed treatment, capture new patient photos, and if time complete a cleaning if indicated.
Insurances that we DO NOT participate with: Medicaid, Humana Medicare, Delta UPHP, Delta Healthy Kids, Federal BCBS Basic, BCBS EPO
Any questions regarding billing and/or insurance coverage should be directed to the front desk.
